The plaintiffs sought to represent a class consisting of "all residents of Mansfield who are being or will be deprived of their constitutional rights under the Fifth, Eighth, and Fourteenth Amendments to adequate medical, psychiatric, and psychological care because of the grossly inadequate medical staff and facilities" at the Ohio State Reformatory in Mansfield, Ohio and the Correctional Medical Center (CMC) in Columbus, Ohio. The class consisted of approximately 2,500 individuals.

In Mansfield, the plaintiffs stated that up to 300 people seek medical care from two part-time physicians who work for two hours every other day. Although there was a staff of four nurses, there was no licensed pharmacist or psychiatrist. The plaintiffs even alleged that incarcerated individuals were occasionally used to perform minor medical operations like suturing, taking x-rays, or administering injections.

If an individual's medical problems could not be resolved in the Mansfield facility, they were transported to the CMC. The CMC was intended to serve all 11,000 people that were incarcerated in Ohio despite holding only 70 beds. The plaintiffs alleged that there were often lengthy delays in scheduling trips to the CMC and inadequate treatment once a trip was scheduled. The plaintiffs alleged that these lengthy delays and inadequate treatment caused severe and sustained suffering, violating their Fifth and Fourteenth Due Process rights as well as their Eighth Amendment right to be free from cruel and unusual punishment. They sought a preliminary and permanent injunction ordering the defendants to "provide adequate medical care in a timely fashion" and to "cease and desist from preventing plaintiffs, by harassment and otherwise, from obtaining adequate care." The also sought declaratory relief and attorneys' fees.

The parties proposed a consent decree and on June 28, 1982, Judge Frank Battisti approved it. The final approved class consisted of "all persons who are or will be inmates of the institution currently designated as the Ohio State Reformatory in Mansfield, Ohio." The consent decree ordered the defendants to provide constitutionally adequate standards of care. Specifically, in terms of staffing, the defendants were ordered to provide a minimum of 34 hours per week of primary on-site physician coverage, increase the number of nurses employed, and hire at least one full-time registered pharmacist, two full-time pharmacy assistants, and four full-time psychologists. Due to the lengthy delays at the CMC, the consent decree required the defendants to use inpatient and outpatient treatment options within community hospitals until there was availability at the CMC. Incarcerated individuals were prohibited from engaging in treatment services. Prison guards were also required to take training courses in order to provide basic medical care.

For many years the consent decree was uncontested and, by all accounts, was complied with by both parties. In 1990, the Ohio State Reformatory in Mansfield was closed in part due to a consent decree entered into in Boyd v. Denton. Then, in 1991, the defendants filed a motion to dismiss due to mootness. The plaintiffs countered in 1992, filing their own motion seeking to enforce the consent decree. The parties litigated the motions in the ensuing years as the case was transferred between a few different judges.
